Fatigue, stress, reduced sexual desire and changes in body composition can have many causes. Symptoms alone do not establish testosterone deficiency, and aging does not automatically create a need for treatment. Start with a physician evaluation rather than a promise to optimize every hormone level.
Diagnosis requires symptoms plus consistently low testosterone on separate morning tests. Free testosterone and additional hormone or safety tests are selected according to the clinical findings. A large laboratory panel is not a substitute for interpreting results in context.

TRT does not guarantee better stress tolerance, mental focus or happiness. New irritability or mood symptoms deserve evaluation rather than being automatically attributed to estradiol. Estradiol has normal functions in men, and estrogen-lowering medication is not routinely required for everyone on TRT.
Medication changes should be based on the person's symptoms, testing and overall treatment plan. Do not add an aromatase inhibitor or change a hormone dose without the prescriber.
Some men with confirmed deficiency experience improved sexual symptoms or lean mass. Other outcomes vary. Follow-up should evaluate benefit and adverse effects, including hematocrit and blood-pressure changes. External testosterone can suppress sperm production, so current or future fertility plans need discussion before treatment.
Clinical supervision helps manage risks but cannot guarantee that side effects will be avoided. Symptoms that persist despite appropriate levels may require a different explanation or treatment.
