Evaluating a testosterone program

TRT is a medical treatment for appropriately diagnosed testosterone deficiency, not a way to turn back time. A program should explain its diagnostic process, intended benefits, risks, follow-up and costs before treatment starts.

What benefits are realistic?

The AUA testosterone deficiency guideline describes potential improvements in sexual symptoms, anemia, bone density, lean mass and depressive symptoms. Evidence is less certain for energy, fatigue, cognition and overall quality of life. A list of possible benefits should not be presented as a prediction for every patient.

Changes in lean mass are not identical to muscle growth, and neither muscle gain nor fat loss can be promised in a fixed number of pounds over six months. Body mass index measures weight relative to height; it does not directly measure body composition.

Is TRT completely safe within a normal range?

No. Appropriate levels and monitoring do not eliminate adverse effects. Testosterone can raise hematocrit or blood pressure, suppress sperm production and cause other problems. A physician should review fertility plans, health history and formulation-specific risks.

Improving bone density is not proof of fracture prevention. TRT should not be presented as a treatment for obesity, a cardiovascular prevention strategy or a guarantee of a longer life.

How progress should be reviewed

Track the symptoms that led to evaluation, relevant laboratory findings and any adverse effects. Different outcomes have different timelines, and some symptoms may not improve. Discuss whether continuing treatment is worthwhile if appropriate testosterone levels do not produce meaningful benefit.
